I agree to join the Indian Pancreatitis Study Group.  I agree to participate in the National Data Collection plan, as per the proforma agreed by all the members of the group.  I understand the following: All investigators who contribute will be acknowledged.  No publication or presentation will be made or results submitted unless all the co-workers agree and sign.  This does not include clinical data exclusive to the particular centre, which the centre has obtained of its own accord and not as part of study.  I will have full access to my data. I will publish or present the data only after all the co-authors give written consent.

I understand that the Amrita Institute has designed a website for the study or data entry.  I agree to make all arrangement for the entry of data on the aforementioned website. In case I do not have facilities for data entry, I will send a printed copy by post to the coordinating centre that will arrange for the data entry.

I understand that before  any research project,  which pertains to the whole or part of the study group, is submitted for potential founding, I must obtain the written consent of all the investigators.
